The Medical Assistant, LPN or RN - Relief on Call position involves providing quality nursing care to clients in accordance with established agency and program guidelines. The role includes performing initial and ongoing health assessments, assessing patient integrated healthcare needs, and identifying any barriers to accessing care. The individual will assess situations and use sound judgment in applying medical or emergency intervention techniques, properly administer medication, document results, and take vital signs on clients. Monitoring patients for changes in health status after medication changes or any assessed decline in function is also a key responsibility. The position requires monitoring lifestyle factors affecting health and assisting clients with goal setting to achieve behavioral change, as well as providing client education specific to client age and condition. Effective communication with clients, peers, professionals, and community agencies regarding client needs is essential. The role also involves arranging medical appointments, making referrals, coordinating with community medical providers, and handling verbal and/or physically abusive clients using de-escalation techniques. Timely and accurate documentation of client care in the agency's EMR is required, along with confirming client payer sources and obtaining authorizations when necessary.
