About The Position

The Medical Assistant (MA) Integrated Behavioral Health (IBH) Care Coordinator is responsible for attending to patients in the THRIVE Program setting for behavioral interventions for a variety of mental health issues, for which the patient would benefit from behavior changes, coaching, or additional support. The MA IBH Care Coordinator contributes equally to the patient care teams through huddles, case consultation, patient advocacy, mental health education, and collaborative team meetings with THRIVE staff.

Requirements

  • High School Diploma or equivalent.
  • Must have a Medical Assistant certificate of completion from a training program.
  • Current CPR or CPR/AED certification required or willingness to complete within 90 days from date of hire.
  • Experience and/or education in human-related fields such as social work, psychology, counseling, alcohol or chemical dependency.
  • Demonstrated commitment to providing services for the underserved and sensitive, working with various people from criminal justice-involved low-income populations with diverse educational, lifestyle, ethnic, and cultural origins; disabled, homeless, substance users, HIV (AIDS) infected, and/or physiologically impaired.
  • Communicate clearly and concisely, both verbally and in writing, and demonstrate high listening skills.
  • Ability to establish and maintain positive, professional relationships.
  • Exceptional interpersonal skills.
  • Excellent customer service experience, problem-solving, and calmness in emotional or stressful situations.
  • Commitment to HIPAA and ability to abide by standards of professional ethics and maintain confidentiality.
  • Ability to maintain detailed records.
  • Compliance with all state and federal laws and regulations pertaining to the position, including Computer (PC) literate in Microsoft Word, Excel, PowerPoint, Outlook email and database software.

  • Adherence to infection control procedures, including but not limited to, standard precautions of temperature monitoring, hand washing, symptom self-monitoring, masking and social distancing.
  • Assist and support the Center/Department/Program to meet standards of High Reliability.
  • Must be willing to successfully complete the credentialing process.

  • Bilingual in Spanish preferred; or other languages desirable.

Responsibilities

  • Perform medical assistant duties, prepare rooms, and room patients, record medical histories, vital signs, perform immunizations, and medication assistance.
  • Schedule and assist patients with follow up appointments.
  • Provide psychoeducation and case management follow-up and referral to improve patient health outcomes.
  • Monitor and implement Certified Community Behavioral Health Center (CCBHC) goals.
  • Monitor and implements Patient-Centered Health Home (PCHH) goals.
  • Assist patients in the navigation of appropriate linkage to various county agencies, and other community resources based on patient need and availability.
  • May intervene for crisis response or to support the patient and care teams in establishing behavioral norms for the continued care relationship.
  • Support the Behavioral Health Psychiatry providers, performing vitals and conducting case management around medication monitoring, behavioral activation, maintenance, and linking patients with resources in the community.
  • Document all patient interactions in the Electronic Health Record according to WellSpace Health policies and timelines.
  • Assist with related data collection, related projects, initiatives, utilization, audits, and review activities.
  • Travel and support transportation when necessary to support operational needs, meetings, and any function requiring presence.
  • Maintain immunization files for the county registry.
  • Maintain and adheres to confidentiality and privileged communications (patient, employee, and organization).
  • Attend required collaborative meetings and trainings as necessary.
  • Perform other duties as assigned within the medical assistant's scope of practice.
