About The Position

The House Calls Medical Assistant/CIS at Oak Street Health plays a vital role in delivering high-quality primary care to Medicare patients, both in neighborhood centers and at home. This position involves supporting logistics, clinical care, and documentation within an innovative home-based care program, working closely with a nurse practitioner to address patient needs and ensure continuity of care.

Requirements

• At least one year of experience as a Medical Assistant with MA certification
• Ability to pass all clinical competencies before supporting patient care; phlebotomy experience is a plus
• Daily access to a vehicle with valid license and insurance
• Flexibility to travel around the Chicagoland area
• Bilingual in English and Spanish is a plus
• US work authorization

Responsibilities

• Preparing visit schedule, route, and logistics for all visits
• Accompanying nurse practitioner on all house calls
• Taking vital signs, performing medication reconciliation, and providing patient/family education
• Documenting House Call notes and managing referrals, orders, and follow-up
• Maintaining supplies and equipment
• Performing other duties as assigned
