About The Position

As a Medical Assistant Care Navigator, you will work collaboratively with a multidisciplinary team to support the delivery of quality healthcare through a Ryan White funded grant program for patients living with HIV. You will Identifiy and resolve barriers to care, provide administrative support, and clinical care as delegated by licensed clinical staff within the scope of a Medical Assistant. This position typically works at the PHC University Campus in Des Moines and is typically scheduled to work 40 hours per week: Monday - Friday 8 AM - 5 PM.

Requirements

  • High school diploma or equivalent.
  • One year of experience in a medical assistant, patient care technician, certified nurse assistant, phlebotomy, or related clinical position or completion of medical assisting program from an accredited institution.

  • Licenses & Certifications: Current Basic Life Support (BLS) for Healthcare Providers; Current Mandatory Reporter Training: Child Abuse and Dependent Adult Abuse recognized by the State of Iowa
  • Must be obtained within the Introductory Period if not current.

Nice To Haves

  • Work experience in a community health center or medical clinic.
  • Effective verbal and written communication skills in Spanish.
  • Phlebotomy skills.

Responsibilities

  • Collaborates with the assigned patient care team, Referral Coordinators, and external service providers to coordinate care for high-acuity patients (e.g. low health literacy, transportation barriers, language barriers, etc.) living with HIV. Identifies and problem-solves barriers that prohibit or delay patients from receiving necessary care.
  • Addresses patient inquiries and care needs both in clinic and through patient communication channels (phone, EMR messaging, etc.), collaborating with appropriate care team members as needed.
  • Coordinates assigned referrals to outside service providers and completes necessary prior authorizations as needed.
  • Communicates with referral coordinators, service providers, HIV care teams to coordinate care for high- acuity patients (e.g. low health literacy, transportation barriers, language barriers, etc.)
  • Facilitates the patient rooming and intake process and obtains accurate patient vital signs per established workflow. Prepares for exam/procedure and assists provider as requested.
  • Performs point-of-care testing per CLIA guidelines and other established workflows. Ensures service orders are completed timely. Administers medications, immunizations, and treatments per written or standing orders.
  • Documents service units and patient data in EMR based on grant requirements and PHC policies & Procedure.
  • Completes and accurately documents quality controls per established process. Performs pre-cleaning and transport of contaminated instruments per infection control policies and procedures. Routinely evaluates supply levels in exam rooms, removing expired supplies and restocking as appropriate.
