About The Position

The Medical Assistant - Care Connections will serve as a vital communication link between Molina Healthcare members and their healthcare providers. This remote position focuses on ensuring members receive quality care by following up after appointments, coordinating care, and assisting with administrative tasks. The role requires effective communication skills to relay important health information and facilitate care coordination, ultimately aiming to improve health outcomes for members.

Requirements

  • High School Diploma or GED required.
  • 1-3 years of clinical experience in family practice, in-home health, primary care, palliative care, or hospice settings.
  • Basic understanding of patient care, medical terminology, coding procedures, and clinical pharmacology relevant to medical assistant practice.
  • Working knowledge of desktop software applications (e.g., Outlook, Word, Excel, Internet, Email).
  • Ability to lift up to 50 lbs.
  • Excellent customer service and communication skills (written & verbal).
  • Ability to work independently or as part of a team.
  • Strong attention to detail and ability to multi-task and prioritize tasks.
  • Critical thinking skills to identify issues and problem-solve effectively.
  • Empathy for working with senior, disabled, and vulnerable populations.
  • Ability to demonstrate positive working relationships with peers and manage conflict.

Nice To Haves

  • Experience as a Certified Medical Assistant for more than 1 year.
  • Experience with electronic medical record systems, preferably Epic.
  • Experience working with underserved populations facing socioeconomic barriers to healthcare.
  • Bilingual in Spanish, Korean, Mandarin, Cantonese, or Vietnamese.

Responsibilities

  • Follow up with members after appointments to ensure quality care and health outcomes.
  • Process and follow up on case management and primary care provider referrals for at-risk patients.
  • Escalate patient issues related to prior authorizations for medications and durable medical equipment.
  • Communicate lab results and exam findings to members and Primary Care Providers as needed.
  • Assist with follow-up calls to ensure positive member experiences and that patient needs are met.
  • Communicate with Nurse Practitioners regarding member health issues for timely follow-up.
  • Facilitate care coordination by sending electronic messages and faxes to primary care physicians.
  • Prepare patient lab result letters and follow up on lab results to close gaps in healthcare.
  • Fulfill nurse practitioner supply orders and maintain nursing supplies inventory.
  • Perform administrative tasks including telephone and email inquiries, follow-up documentation, and patient record updates.
  • Assist with special projects under the direction of the Clinical Integration leadership team.
