About The Position

The Medical Assistant - Care Connections will serve as a vital link between Molina Healthcare members and their healthcare providers, ensuring that members receive quality care and follow-up after appointments. This role involves communication with members regarding their health outcomes, coordination of care, and performing various administrative tasks to support patient care and documentation.

Requirements

  • High School Diploma or GED required.
  • 1-3 years of clinical experience in family practice, in-home health, primary care, palliative care, or hospice settings.
  • Basic understanding of patient care, medical terminology, coding procedures, and clinical pharmacology relevant to medical assistant practice.
  • Working knowledge of desktop software applications (e.g., Outlook, Word, Excel, Internet, Email).
  • Ability to lift up to 50 lbs.
  • Excellent customer service and communication skills (written & verbal).
  • Ability to work independently or as part of a team.
  • Strong attention to detail and ability to multi-task and prioritize tasks.
  • Critical thinking skills to identify issues and problem-solve effectively.
  • Empathy for working with senior, disabled, and vulnerable populations.
  • Ability to demonstrate positive working relationships with peers and manage conflict.

Nice To Haves

  • Experience as a Certified Medical Assistant for more than 1 year.
  • Experience with electronic medical record systems, preferably Epic.
  • Experience working with underserved populations facing socioeconomic barriers to healthcare.
  • Bilingual in Spanish, Korean, Mandarin, Cantonese, or Vietnamese.

Responsibilities

  • Follow up with members after appointments to ensure quality care and better health outcomes.
  • Assist at-risk patients by processing and following up on case management and primary care provider referrals.
  • Escalate patient issues related to prior authorizations for medications and durable medical equipment.
  • Communicate lab results and other exam findings with members and Primary Care Providers as needed.
  • Make follow-up calls to members to ensure positive experiences and that patient needs are met.
  • Communicate with Nurse Practitioners regarding member health issues for timely follow-up.
  • Facilitate care coordination by sending electronic messages/faxes to primary care physicians.
  • Prepare patient lab result letters and follow up on lab results to close gaps in healthcare.
  • Fulfill nurse practitioner supply orders and maintain nursing supplies inventory.
  • Perform administrative tasks such as handling telephone and email inquiries, follow-up documentation, and updating patient records.
  • Assist with special projects under the direction of the Clinical Integration leadership team.
