Our Care Providers provide daily support to individuals with developmental disabilities within the home setting. Working with a team, Medical Appointment Coordinators (MAC’s) are responsible for the coordination of all medical appointments and all necessary communication and documentation as needed to link individuals who live in residential sites to proper medical care. We are a parent and family driven organization that empowers and supports people of all ages with a broad spectrum of emotional, intellectual and developmental disabilities. WHAT WOULD I BE DOING? Responsible for assisting people supported in receiving needed/desired medical services and communicating and documenting pertinent information as per program policy (i.e., medication administration, transportation to appointments). Ensures an adequate amount of medication is on site for each individual at all times. Provides communication to residential site staff to ensure physician and clinical orders are understood and followed. Makes recommendation to the CSRN and/or Health Care provider to maintain and/or improve the overall health and wellness of individuals supported. Completes required paperwork on daily / as needed basis. Provides direct support as needed. Provide transportation as needed with agency vehicles. Use various software platforms for specific job functions.
