About The Position

As an Advanced Practice Provider (NP/PA) at our Street Medicine Clinic, you will provide comprehensive, patient-centered primary care to individuals and families across the lifespan. You will deliver high-quality medical services, including health assessments, diagnosis and treatment of acute and chronic conditions, preventive care, and patient education for our diverse patient population. Requirements

  • Physician Assistant: A medical license from the State of Idaho to practice medicine as a Physician Assistant under a Physician’s supervision plus a medical certification from the National Commission on Certification of Physician Assistants Inc. (NCCPA) is required. The medical license must be free of restriction and no litigation or disciplinary actions may be pending against the incumbent.
  • Nurse Practitioner: Certified as a Family Nuse Practitioner (FNP) or Advanced Registered Nurse Practitioner (ARNP) in the State of Idaho or qualified to receive certification as an FNP or ARNP in the state of Idaho. State of Idaho RN or ability to receive licensure required.
  • Must have or ability to obtain Basic Life Support (BLS) certification, DEA and State of Idaho Controlled Substance Registration numbers.
  • 2 or more years of experience as a primary care provider is preferred.

Responsibilities

  • In accordance with standard of care and within scope of practice, provides primary care to patients through diagnosis, treatment, and management of acute and chronic problems in accordance with standards of care as described in Principles of Practice, protocols, and scope of practice as approved by supervising physician and Board of Medicine.
  • Consult with other clinic providers, specialists, and health care professionals as appropriate.
  • Supervises or coordinates the activities of patient care and support staff.
  • Collaborates with other health care providers as part of a multidisciplinary team to provide comprehensive patient care.
  • In coordination with other clinic staff, maintains system of quality assessment and assurance in accordance with quality assurance plan.
  • Participates in program planning, development, implementation and evaluation relating to specific job, clinic, and program functions.
  • Participates in staff meetings, conferences, and in-service training and continuing education.
  • May be asked to participate in a shared call system with other providers.
  • Responsible for accurate, timely, and complete documentation in patient records. Including providing appropriate code level and diagnosis of patient visits using CPT & ICD-10 coding standards.
  • Counsels and educates patients and family members regarding health maintenance, disease prevention, diagnosis, treatment, and need for follow-up as appropriate for each visit. Verify and document the patient’s understanding of diagnosis, treatment and follow-up recommendations.
  • Perform minor procedures and in-house lab work in accordance with Heritage Health policy and qualifications.
  • Practices safety, environmental, and/or infection control methods.
  • Participates in QA/QI activities.
  • May be required to work with frequent interruptions and respond appropriately to unexpected situations.
  • Heritage Health staff have an active role in our Patient Centered Medical Home model of care. This role is designated as part of the Heritage Health PCMH Care Team.
  • Regular and predictable attendance is an essential function of this position.
  • Performs miscellaneous job-related duties as assigned.
