About The Position

Provides direct and indirect clinical social work and discharge planning services for patients and their families in various inpatient and outpatient settings. Participates as a professional team member in patient care planning along with other hospital and community agency personnel.

Requirements

• Master’s Degree in Social Work required.
• Licensure with the State of Ohio Counselor, Social Work and Marriage and Family Therapist Board. LSW required, LISW preferred.
• Previous experience in a health care setting or other social service Agency required.
• Must possess strong clinical, interpersonal skills.
• Must possess strong communication skills, both in verbal skills and in writing skills.
• Must possess ability to work in a busy, stressful environment, and be able to work independently.
• Basic Computer Skills required.

Nice To Haves

• 3-5 years of experience preferred.
• Current knowledge of medical terminology, Medicare/Medicaid and third party payor requirements preferred.
