Make a Difference U nder the general supervision of the supervisor, is responsible for supporting the clinical activities of the ambulatory care pharmacy department including program management and coordination of patient care follow up.

Requirements

  • High School Diploma or GED is required (Required)
  • Indiana Pharmacy Technician License or Medical Assistant Certification or AAMA Credentialing is required.
  • Required CertIfications/Licensures\: (Pharmacy Technician Certification Board (PTCB) National Certification)
  • 0+ years\: Previous pharmaceutical or related healthcare experience education preferred (Required)

Responsibilities

  • Communicates directly with patients to discuss their needs and coordinate follow up. All interaction will be documented in EPIC.
  • Coordinates responses and resolutions to issues with appropriate internal and external parties.
  • Provides persistent and timely follow up directly and effectively with the other party in writing, in groups, electronically and on the phone including patients, pharmacist and other providers.
  • Maintains current documentation related to the patients drug therapy and pharmacy care plan.
  • Flag issues that need to be addressed .
  • Develop and maintain databases to proactively drive patient care follow up.
  • Correctly evaluates and identifies patient and program needs through self-driven evaluation of the situation.
  • Ensures a continuous quality improvement customer service approach by proactively identifying areas of improvement and communicating those ideas to the healthcare team.
  • Maintains proficiency with departmental information system technology and documentation systems as well as contemporary professional knowledge base.
  • Provides direct program coordination to the Bridges to Health program, the clinical pharmacist disease state management programs, and the clinical pharmacist Medicare Part D program sessions.
  • Develops, implements, and oversees the yearly Medicare Part D sessions through the network.
  • Supports other team members in the healthcare team.
  • Completes mandatory learning, licensing, and other requirements independently, without the intervention of leadership.
  • Supports other team members on the healthcare team.
  • Follows all network compliance policies, the HIPAA Privacy Rule and the Network Responsibility & Compliance Program (NRCP), which includes the Code of Conduct.
  • Performs all duties respectful of confidentiality and safety of all patients, employees, and physicians.
  • Adheres to established quality principles and standards.
  • Performs related duties as assigned.
