About The Position

Smoky Hill is seeking an experienced MDS/Wound Care Nurse to join our team! 1007 Johnstown, Salina, KS 67401 5,000 SIGN ON BONUS LVN or RN MDS/Wound Care Nurse: Job Responsibilities: Coordinate Resident Assessments: Oversee admission, annual, quarterly, Medicare, and change-of-condition assessments, while ensuring timely communication with nursing and other departments. Ensure Regulatory Compliance: Follow all Federal and State guidelines for comprehensive assessments (MDS). Monitor Documentation: Review MDS and care plan files for accuracy, thoroughness, and organization. Utilize MDS Software: Maintain expertise in software systems to ensure correct data entry and reporting. Lead Interdisciplinary Meetings: Attend and coordinate Medicaid, ITTM, and care plan meetings for smooth team communication. Update Care Plans: Adjust and revise care plans as needed based on current assessments and resident status. Provide Quality Reporting: Generate and distribute Quality Indicators Reports to leadership to support care improvement. Submit MDS Data: Ensure timely and accurate transmission of MDS assessments per reporting standards. Stay Current: Attend professional development sessions to keep up with industry standards and best practices. Oversee Ancillary Data: Collect and report ancillary services to corporate billing for accurate processing. Wound Care Duties: Provide hand on care to the resident based on the resident Plan of Care.  Administer medications and provide treatment to the resident based on the physician orders.  Must be able to identify changes in the resident condition and evaluate the resident care needs.  Strong focus assessment skills as well as documentation skills.  Must be able to evaluate and participate in the development of the Resident Plan of Care.  Monitor the resident overall condition and provide care as required while maintaining compliance with the facility policy and procedures.  Provide primary skin care to residents under the medical direction and supervision of the residents’ attending physicians, or the Director of Nursing Services, with an emphasis on treatment and therapy of skin disorders.  Must be able to identify, manage, and treat specific skin disorders, primary and secondary lesions, and decubitus ulcers.  Ensure that residents with decubitus ulcers received appropriate prophylaxis and treatment such as daily inspection, turning and activity, a well-planned diet, and maintaining a clean, dry bed.  Must be able to push, pull, moved and/or lift a minimum of 25 pounds and/or carry such weight a minimum distance of 50 feet.

Requirements

  • Must have 2 years of long term care experience
  • Must have MDS / Wound Care experience
  • Must be fluent in MDS 3.0
  • Must possess a current, unencumbered, active license to practice as a Registered Nurse or Licensed Vocational Nurse in this state.
