Requirements

  • A Current RN or LPN license in the state of Michigan is required.
  • A minimum of two (2) years of proven experience in MDS coordination within a skilled nursing or similar healthcare setting.
  • A strong, in-depth understanding of the entire MDS assessment process and all related state and federal regulations.
  • Exceptional communication and interpersonal skills, allowing you to effectively collaborate and build rapport.
  • A demonstrated ability to work effectively and contribute positively in a dynamic team environment.
  • Excellent problem-solving and decision-making skills, with an eye for detail and accuracy.

Responsibilities

  • MDS Assessment Mastery: Expertly coordinate and complete all MDS assessments with precision and punctuality, ensuring full compliance with state and federal regulations.
  • Interdisciplinary Collaboration: Work seamlessly with our diverse interdisciplinary team (nurses, therapists, social workers, etc.) to develop comprehensive and truly individualized care plans that meet each resident's unique needs.
  • Resident & Family Communication: Clearly and compassionately communicate assessment results and care plan details to residents and their families, fostering transparency and involvement in their care journey.
  • Quality Improvement Champion: Actively participate in ongoing quality improvement initiatives, contributing your expertise to enhance our MDS assessment processes and overall resident outcomes.
