Requirements
- Valid NYS LPN license
- Experience with MDS (preferred) or willingness to learn
- Strong attention to detail and clinical judgment
- Excellent communication and teamwork skills
Nice To Haves
- Prior experience in skilled nursing or long-term care a plus
Responsibilities
- Completing and coordinating timely MDS assessments and submissions
- Collaborating with the interdisciplinary team on resident care plans
- Ensuring documentation meets regulatory and quality standards
- Tracking resident changes and updating care plans accordingly
- Supporting audit readiness, case-mix accuracy, and survey prep
