About The Position

MDS Nurse (RN or LPN) Location: Colonial Vista Post Acute Pay: $32–$54 per hour (based on experience and licensure) Colonial Vista Post Acute is seeking an exceptional MDS Nurse (RN or LPN) to join our leadership team. Requirements

• Active RN or LPN license in good standing
• MDS experience preferred; strong clinical background required
• Working knowledge of Medicare, Medicaid, PDPM, and regulatory requirements
• Strong assessment and documentation skills
• Ability to lead, collaborate, and hold high standards

Responsibilities

• Complete and coordinate MDS assessments in accordance with federal and state regulations
• Ensure accuracy of coding and appropriate reimbursement optimization
• Collaborate with IDT to capture clinical documentation that reflects the true acuity of our residents
• Participate in care plan development and interdisciplinary meetings
• Monitor regulatory compliance and support survey readiness
• Educate and support nursing staff related to documentation and MDS processes
