Requirements
- Current RN licensed from the State of Illinois
- RAC-T Certified required
- Knowledge of Minimum Data Set (MDS), ICD-10 Coding
- Two (2) to three (3) years of applicable experience as a Nurse preferred
- Must be CPR Certified
- Experience in a long term care setting preferred
Nice To Haves
- American Association of Nurse Assessment Coordination (AANAC) certification preferred
Responsibilities
- The MDS Coordinator is responsible for leading the Interdisciplinary Team Plan
- Organize, develop and implement the MDS assessments and care plans
- Update records and assessments, ensuring information is consistent with patient care plans and that scoring is accurate
- Accountable for accurate MDS scoring
