About The Position

Diablo Valley Post-Acute is seeking a meticulous, leadership-driven MDS Nurse (RN) to oversee our resident assessment process. In this vital role, you will ensure that all assessments—including MDS, individual assessments, and resident plans of care—are completed with the highest degree of accuracy and in strict accordance with federal and state guidelines. Requirements

  • Licensure: Must be a Registered Nurse (RN) in good standing with the State Board of Nursing.
  • Education: Graduation from an accredited School of Nursing (BSN preferred).
  • Experience: Strong preference for Long-Term Care (LTC) experience.
  • 3 years of supervisory experience preferred.

Responsibilities

  • PDPM Oversight: Lead weekly PDPM meetings for skilled patients and monitor the effective utilization of tracking tools (Physician Diagnosis Verification, CSR(GG), Weekly CSR, Point Right, etc.).
  • Assessment Mastery: Ensure the accurate and timely completion of all MDS Assessments, including PPS Medicare, quarterly, annual, and significant change assessments.
  • Quality Analysis: Track and monitor QM and QASP performance monthly and quarterly, providing clinical recommendations to optimize facility outcomes.
  • Interdisciplinary Coordination: Lead the interdisciplinary team in completing the Minimum Data Set for every new admission and maintain accurate schedules for all assessments.
  • Care Plan Development: Guide all disciplines in formulating, evaluating, and revising comprehensive, individualized care plans that prioritize realistic resident goals.
  • Data Integrity: Oversee electronic submission of MDS assessments, verify submissions, perform necessary corrections, and maintain all mandated records.
  • Care Conferences: Schedule and conduct resident care conferences in compliance with state and federal regulations.
  • Compliance: Manage all level-of-care changes and ensure proper notification to all departments and state agencies as required by law.
