MDS Coordinator RN or LPN - Full-Time – Generations at Applewood, Matteson, IL Generations at Applewood in Matteson, IL, has an immediate opening for a MDS Nurse (RN or LPN) Salary is $90k-95k depending if a RN, MDS Certification and experience. Duties of the MDS Nurse: The MDS Coordinator is responsible for leading the Interdisciplinary Team Plan, organize, develop and implement the MDS assessments and care plans Update records and assessments, ensuring information is consistent with patient care plans and that scoring is accurate Accountable for accurate MDS scoring Generations at Applewood is a 24-hour rehabilitation and skilled nursing community offering quality healthcare services to senior residents throughout the Chicagoland area. Requirements
- Current RN or LPN licensed from the State of Illinois
- Knowledge of Minimum Data Set (MDS), ICD-10 Coding
- Two (2) to three (3) years of applicable experience as a MDS Nurse preferred
- Must be CPR Certified
- Experience in a long term care setting
Nice To Haves
- American Association of Nurse Assessment Coordination (AANAC) certification preferred
- RAC-T Certified preferred
