Responsibilities

  • Oversees weekly PDPM meetings for skilled patients and monitors facility use of PDPM tools such as Physician Diagnosis Verification, CSR(GG), and Point Right.
  • Tracks and analyzes Quality Measures (QM) and QASP performance monthly and quarterly, providing clinical recommendations for optimal outcomes.
  • Provides comprehensive PDPM and MDS training to new MDS Coordinators to ensure regulatory compliance and data accuracy.
  • Ensures accurate and timely completion of all MDS assessments, including PPS Medicare, quarterly, annual, and significant change evaluations.
  • Coordinates interdisciplinary participation in the completion of MDS and resident care conferences within required regulatory time frames.
  • Reviews and evaluates care plans for completeness, accuracy, and effectiveness, ensuring individualized resident goals and interventions.
  • Manages all level-of-care changes, notifies departments, and transmits required documentation to appropriate agencies.
  • Maintains data integrity through accurate entry, electronic submission, and correction of MDS assessments as needed.
  • Protects resident confidentiality and ensures all documentation meets federal, state, and medical standards.
  • Supervises assigned staff and ensures all licensing, continuing education, and professional standards are maintained in compliance with state nursing regulations.
