About The Position

The MDS Nurse-LVN is responsible for overseeing the facilities' weekly PDPM meetings for skilled patients and tracking the effective utilization of PDPM tools. This includes monitoring QM and QASP performance and providing data analysis and clinical recommendations for optimal outcomes. The role involves providing PDPM and MDS training to new MDS Coordinators, assessing the health status and level of care of new admissions, and ensuring the accurate and timely completion of all MDS Assessments. The MDS Nurse coordinates interdisciplinary participation in completing the Minimum Data Set (MDS) for each new admission and ensures compliance with federal, state, and medical standards. The position also includes scheduling resident care conferences, formulating care plans, and managing level of care changes within the facility.

Requirements

  • Graduation from an Accredited School of Nursing.
  • Bachelor of Science Degree in Nursing preferred.
  • Good standing with the State Board of Nursing.
  • 3 years of supervisory experience preferred.
  • Long Term Care Experience preferred.
  • Proficiency in Microsoft Suite products.
  • Knowledge and experience with PCC preferred.

Responsibilities

  • Verify electronic submissions and maintain records.
  • Schedule and conduct resident care conferences.
  • Assist in formulating and revising care plans.
  • Evaluate resident care plans for comprehensiveness.
  • Manage level of care changes and notify departments.
  • Maintain confidentiality of resident care information.
