About The Position

The Minimum Data Set (MDS Coordinator) is critical in health care and is responsible for the MDS of short-term stay patients in Transitional Care. This position is responsible for the assessment and documentation process included in the MDS required by the Centers for Medicare and Medicaid Services. The MDS Coordinator will monitor MDS coding for accuracy and completion, as well as Care Area Assessment (CAA) management and Care Planning. Additionally, this position is dedicated to expanding the integration of MDS critical thinking, awareness, and competency into the care plan process. The MDS Coordinator can be a Registered Nurse (RN) or Licensed Practical Nurse (LPN) responsible for accurately assessing, documenting, and coordinating care for residents in nursing care facilities. This role involves collecting patient data to create a comprehensive care plan and ensuring compliance with state and federal regulations, particularly for Medicare and Medicaid reimbursement purposes. The MDS Coordinator also works collaboratively with the supportive living services line to establish system delivery that supports VMRC's mission, vision, and values.

Requirements

  • Must be a Registered Nurse (RN) or Licensed Practical Nurse (LPN).
  • Experience in long-term care or skilled nursing facilities preferred.
  • Knowledge of MDS processes and regulations.
  • Strong communication and interpersonal skills.
  • Ability to work collaboratively with interdisciplinary teams.
  • Attention to detail and strong organizational skills.

Responsibilities

  • Participates in overseeing all facets of Complete Living and Transitional Care to adhere to industry regulations and VMRC's program philosophy of service and care.
  • Performs comprehensive assessments of residents using the MDS tool to gather accurate data related to physical, cognitive, emotional, and functional health status.
  • Ensures MDS assessments are completed promptly within the timeframe CMS requires, including entry, admission, annual, quarterly, significant change, optional state assessments, and discharges.
  • Completes other MDS assessments to maintain compliance with federal, state, and local standards.
  • Works with the billing and finance department to ensure accurate coding and billing is based on the MDS assessments.
  • Participate in a 'triple check' with the billing and finance department to ensure accurate claims are submitted.
  • Ensures that residents' medical records and provider documentation accurately reflect diagnosis codes related to the care provided.
  • Review resident immunization records for accuracy and completion.
  • Monitors regulations and industry standards changes to ensure ongoing compliance with CMS guidelines.
  • Participate and help in the admission process as requested.
  • Ensures the comprehensive resident care planning process is completed according to state, federal, and local standards.
  • Communicate staff training needs to the DON.
  • Participate in the review of resident medical records for proper documentation.
  • Champion resident-centered care.
  • Participates in and completes all organization and service line education and training.
  • Assumes responsibility for and participates in staff development (professional growth) and training to remain knowledgeable of market and industry trends, competitors, and best practices for non-profit CCRCs.
  • Builds effective relationships and encourages the growth and development of others.
  • Maintains confidentiality of all resident care and protected health information.
  • Demonstrates computer skills necessary to complete job tasks/responsibilities.
  • Attends all required in-services and service line staff meetings.
  • Follows emergency and evacuation procedures.
