About The Position

Our team provides quality nursing care for our 132-bed skilled nursing facility. To us, our commitment is more than a job. We are making a difference in the quality of life for our residents and their families. White Oak of Lancaster is seeking an RN to join in our current NAC team. The Nurse Assessment Coordinator recognizes and documents residents' needs and strengths per the Federal and State approved MDS assessment. Utilize Resident Assessment Instrument process information to create a functional Plan of Care to address residents' needs and strengths to be used as an approach for providing quality care; and to help the resident meet his/her mental, emotional, social, and physical needs. This is a full-time salaried position working Monday - Friday day shift. This is an exciting time to be a member of the White Oak of Lancaster team! We have a strong team of exceptional RNs, LPNs, and CNAs, as well as a team of corporate nursing consultants dedicated to ensuring your success. Our nurse managers excel in their roles and provide consistent and valuable support for the NAC Nurses. The ideal candidate is compassionate, caring, flexible and innovative with a strong leadership, critical thinking, problem solving, communication, customer service and organizational skills. Must relate professionally with residents, staff, family members, referral sources, physicians, and vendors. Qualifications: Graduate of a state approved school of Nursing. Current and active RN license in the state of residence. Must be in good physical and mental health and must be kind, considerate, understanding and enjoy working with elderly people.

Requirements

  • Graduate of a state approved school of Nursing.
  • Current and active RN license in the state of residence.
  • Must be in good physical and mental health and must be kind, considerate, understanding and enjoy working with elderly people.
  • Excellent interpersonal and communication skills
  • Proficient computer skills
  • Well-developed leadership and team building skills
  • Current, unencumbered, active license to practice as a RN in SC
  • A minimum of two (1) year RN experience in a long-term care setting with proven leadership and management skills
  • Proficiency of Federal and State Long Term Care Regulations and Procedures, nursing services, rehabilitation therapy, MDS process and resident care

Nice To Haves

  • Knowledge of CMI, Medicare and PDPM evaluations, is a plus
