About The Position

Brockton Home Health AFC is Medicaid certified in Massachusetts and accredited by Commission on Accreditation of Rehabilitation Facilities (CARF) - Accreditation of Healthcare Organizations (CARF). Brockton Home Health Care AFC is looking for A registered Nurse (RN)/Case Manager to manage and support our Adult Family/Foster to make sure the our programs runs smoothly. We are seeking a caring and detail-oriented Registered Nurse to join our Adult Foster Care (AFC) program as an Initial Assessment / Admission Nurse. In this role, you will complete in-home evaluations for individuals applying to the AFC program, reviewing their needs and confirming eligibility. This opportunity is ideal for nurses with experience in geriatrics, home health, or community-based care who are committed to supporting individuals and enhancing quality of life.

Requirements

Demonstrated ability to coach, train and provide staff development
Proficiency in Microsoft (Word, Excel and PowerPoint)
MS or Bachelor’s degree in Nursing
Must be fully licensed RN by Mass Board of Registration
At least five years of previous home health care experience

Responsibilities

Conduct comprehensive in-home assessments for new AFC applicants.
Evaluate medical, functional, and psychosocial conditions to determine eligibility.
Partner with caregivers, families, and healthcare professionals to design customized care plans.
Provide education to participants and families regarding AFC services, expectations, and program policies.
Ensure compliance with all state regulations and AFC program requirements.
Maintain accurate, up-to-date documentation of assessments, care plans, and follow-up activities.
