About The Position

The MDS Coordinator at Regency at Shelby Township plays a crucial role in overseeing the RAI (Resident Assessment Instrument) process, ensuring that assessments and care plans are conducted efficiently and effectively for our guests. This position is designed for experienced MDS nurses who are looking to take the next step in their careers. The MDS Coordinator will supervise the Care Management Nurse and MDS Nurse, fostering a collaborative environment that emphasizes communication and teamwork. In this role, the MDS Coordinator is responsible for completing the MDS, CAA's (Care Area Assessments), and care plans within regulated time frames. This includes coordinating the scheduling of the RAI process with the interdisciplinary team, which consists of various healthcare professionals. The MDS Coordinator will assess residents through physical assessments, interviews, and chart reviews, ensuring that all care needs are thoroughly discussed with caregivers, including physicians, nursing staff, social services, therapy, dietary, and activity staff. Additionally, the MDS Coordinator will review information from hospitals, consults, and outside agencies, utilizing this information to complete assessments and care planning. They will also coordinate and identify significant changes in MDS and be prepared to conduct PPS (Prospective Payment System) meetings, maintaining MDS assessments per Medicare schedules. The MDS Coordinator will also keep the PPS board updated for monitoring Medicare days and RUGs (Resource Utilization Groups) utilization in the absence of the Care Management Coordinator. Staying current with the requirements set by the American Association of Nursing Assessment Coordinators (AANAC) is essential for this role, ensuring compliance and best practices in resident care.
