About The Position

The MDS Coordinator (also known as the Resident Assessment Coordinator, RAC) is responsible for the overall management of the Resident Assessment Instrument (RAI) process, including timely and accurate completion of the Minimum Data Set (MDS) and Care Area Assessments (CAAs). This role ensures compliance with all federal (CMS/OBRA), state, and local regulations to support resident care planning, quality measures, and appropriate reimbursement (Case Mix/RUG-IV/PDPM). As a department head, this role also provides supervision and guidance to other MDS personnel. ~~~~~~~~~~~~~~~~~~~~~~~ Come join our family of caregivers! Requirements

  • Must be a graduate of an accredited school of nursing.
  • Current, unencumbered Registered Nurse (RN) license in the State of Rhode Island (or compact state).
  • Prior experience as MDS Coordinator in a Long-term care facility preferred but will consider candidates with comprehensive knowledge of the position.
  • Demonstrated supervisory and leadership abilities.
  • Must possess exceptional organization, time management, and communication skills.
  • Proficient with electronic health records (EHR) and MDS submission software.

Nice To Haves

  • MDS certification (e.g., RAC-CT) preferred.
