About The Position

The MDS Coordinator is responsible for coordinating, overseeing, and completing the Minimum Data Set (MDS) assessments and care plans for residents in accordance with federal and state regulations. This position ensures that the facility remains in compliance with CMS guidelines while promoting optimal quality of care.

Requirements

  • College degree or equivalent from a two-year college or technical school
  • Six months to one-year experience, preferably in a long-term care setting
  • Experience with MDS and PPS software
  • Active LPN or RN Licensure by the State of Tennessee
  • Attention to detail and accuracy in documentation
  • Excellent organizational and time-management skills
  • Effective communication and interpersonal skills
  • Proficiency in EHR/MDS software (PointClickCare, MatrixCare, etc.)
  • Ability to work independently and as part of a team
  • Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of people or employees of organization.
  • Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
  • To perform this job successfully, an individual should have the ability to gain knowledge of EHR/MDS software, Microsoft Office, and Internet.
  • While performing the duties of this job, the employee is frequently required to stand; walk; use hands to finger, handle, or feel; reach with hands and arms and talk or hear. The employee is occasionally required to sit and stoop, kneel, crouch, or crawl. The employee must regularly lift and /or move up to 25 pounds. Specific vision abilities required by this job include close vision, distance vision, peripheral vision, depth perception and ability to adjust focus.
  • The work environment involves the everyday risks or discomforts that require normal safety precautions typical of such places as offices, meeting and training rooms, etc. The work area is adequately lighted, heated, and ventilated.

Responsibilities

  • Conducts and coordinates the development and completion of the resident assessment (MDS) in accordance with rules, regulations and guidelines that govern the resident assessment, including the completion of RAP’s and care plans.
  • Coordinates with the interdisciplinary care plan team in developing a comprehensive resident assessment and care plan for each resident.
  • Evaluate each resident’s condition and pertinent medical data to determine any need for special assessment activities.
  • Contact and assist in scheduling participation by outside members of the care plan team, including the resident, the resident’s family and/or resident’s representative.
  • Coordinates the development of a written plan of care, preliminary and comprehensive, for each resident that identifies the problems/needs of the resident, indicates the care to be given, goals to be accomplished, and which professional service is responsible for each element of care.
  • Ensures that care plans include measurable objectives and timetables to meet the resident’s medical, nursing, mental and psychosocial needs as identified in the resident assessment.
  • Ensures that all assessments are completed as scheduled in accordance with state and federal regulations.
  • Interacts positively with co-workers, physicians, residents and families
  • All other duties as assigned by the Director of Nursing
