About The Position

The MDS Coordinator is responsible for completing all MDS, admission, significant change, change of therapy, end of therapy, discharge and re-entry tracking forms, as well as quarterly and annual assessments, following the directions provided in the RAI manual, which can vary annually. This role involves entering appropriate MDS data into a patient information system and transmitting it to CMS via the state agency according to CMS timelines. The coordinator must ensure all MDS/CAA records are available for a fifteen (15) month period and that all individuals involved in the RAI process complete their specific MDS sections, CAAs, and Care Plans within CMS-regulated timeframes. Communication with physicians, team members, and other involved individuals is crucial for maintaining accurate assessments through interaction and routine meetings. The position also requires providing input into the care plan process both initially and throughout the resident’s stay, collaborating with the interdisciplinary team for problem-solving and resident-related issues to enhance care, and developing and implementing annual goals that align with those of the team. Other duties may be assigned as appropriate.

Requirements

  • Graduate of an accredited nursing program
  • Active RN license in the state you will be working
  • Excellent communication skills to interact verbally with residents, visitors, and staff
  • Ability to manage time and organize daily schedule to meet productivity standards
  • Ability to stand and walk for long periods of time
  • Ability to lift heavy objects using proper lifting techniques and with assistance as necessary
  • Manual dexterity, coordination and skillful use of hands when working with patients and equipment
  • Ability to physically perform necessary documentation both electronically and manually
  • Ability to work independently with minimal supervision
  • Must be a team player

Nice To Haves

  • Two years previous experience with long-term care plans, MDS state regulations, and CMS related to MDS preferred

Responsibilities

  • Provides input into the care plan process initially and throughout the resident’s stay.
  • Collaborates with the interdisciplinary team for problem solving and any resident-related issues that will enhance the care provided to each resident.
  • Develops and implements specific goals on an annual basis that will coordinate with those of all team members.
  • May be assigned other duties as appropriate.
