About The Position

Complete assessments, interviews and evaluations as required for the Minimum Data Set (MDS). Schedule MDS’s for the residents of the Inpatient Rehab Unit, Neurocare Unit, and Long-Term Care in accordance with RAI guidelines, while optimizing Medicare Reimbursement, Case-Mix Index, and Quality Measures. Generate and distribute MDS schedule and care plan schedule. Monitor progress and completion of MDS components throughout the various disciplines (Nursing, Therapy, Social Services, Dietary, Activities) to ensure timeliness as prescribed by regulatory guidelines. Advise staff in the various disciplines of incomplete and/or untimely assessments, interviews, and evaluations. Participate in care plan creation, updates, and meetings, as necessary. Perform all job duties in a manner that ensures that confidential information and resident rights are always protected. Maintain current knowledge of RAI Manual and MDS assessment process. Participate in facility management and administrative meetings. Monitor, advise, and direct the MDS Coordinator – LPN. Provide coverage for the Nursing department as needed and directed by the DON. Participate in rotating “Administrator on Call” weekend coverage as scheduled. Educate and advise new and existing staff on MDS and RAI processes and updates. Generate and present audits, as assigned. Perform medication prior authorizations as directed by and in coordination with facility medical providers. Access to entire medical record of all patient’s/resident’s. Performs duties according to the highest ethical and moral standards and in adherence with the facility’s corporate compliance plan and applicable governing laws. Adheres to the facility’s confidentiality statement in accessing and safeguarding protected health information (PHI) of residents and employees, as necessary to perform job duties. Performs any other duties assigned by the Administrator and/or designee.

Requirements

  • Graduate of an approved/accredited School of Nursing and currently licensed as a Registered Professional Nurse in New York State.
  • Experience in Long-Term Care or Rehabilitation Services.
  • Knowledge of Point Click Care (PCC) software platform.
  • Qualified as a PRI Assessor.

Nice To Haves

  • Bachelor of Science degree in nursing preferred.
  • RAC-CT certified from an accredited professional organization.
