The MDS Coordinator at Alden Town Manor Rehabilitation and Health Care Center is responsible for overseeing the scheduling, completion, and accuracy of all MDS item sets. This role involves coordinating the development of individualized care plans for residents, ensuring that all documentation is consistent and meets program standards. The MDS Coordinator also chairs care plan conferences, facilitates communication between residents, families, and the administration, and participates in educational programs related to the RAI process.

  • Review the department's policy and procedures annually and make recommendations to the Administrator.
  • Oversee the scheduling, completion, and accuracy of all MDS item sets.
  • Coordinate the development of a written plan of care for each resident, identifying their problems, needs, and concerns.
  • Check for adequate descriptions of problems and ensure resident-centered goals are realistic and measurable.
  • Chair resident care plan conferences and relay concerns from residents and families to the Administrator.
  • Ensure residents and families are contacted regarding participation in care plan conferences per RAI guidelines.
  • Plan, conduct, and participate in In-service Education Programs related to the RAI process.
  • Assist in implementing and maintaining an effective orientation program for new employees regarding the RAI process and data entry.
  • Attend and participate in continuing education programs.
  • Monitor residents' medical records for consistency and content, addressing discrepancies with appropriate managers.
  • Maintain ICD-9 codes for current diagnoses.
  • Report IT concerns to the help desk in a timely manner.
  • Participate in the development, maintenance, and implementation of the facility's QAPI program.
  • Attend relevant meetings as directed by the Administrator.
  • Perform other related duties as assigned.
  • 1 year of experience in care plans and MDS processes.
  • Knowledge of RAI guidelines and care plan development.
  • Ability to monitor and ensure documentation consistency and compliance with program standards.
  • Experience in a rehabilitation or health care setting.
  • Familiarity with ICD-9 coding and medical record management.
  • Strong communication and interpersonal skills.
  • Competitive hourly wage up to $44 per hour.
