About the position

The MDS Coordinator at Shoreline Healthcare Center is a vital role responsible for overseeing the Minimum Data Set (MDS) assessments and ensuring compliance with healthcare regulations. This position involves collaboration with interdisciplinary teams to enhance patient outcomes through accurate documentation and assessment of patient care.

Responsibilities

  • Conduct comprehensive assessments of patients using the MDS framework to gather essential clinical data.
  • Collaborate with nursing staff and other healthcare professionals to ensure accurate and timely completion of MDS documentation.
  • Review and analyze medical records to ensure compliance with regulatory requirements and quality standards.
  • Participate in utilization reviews to assess the appropriateness of care provided to patients.
  • Maintain up-to-date knowledge of changes in regulations related to MDS and patient care practices.
  • Provide training and support to staff on MDS processes, medical terminology, and documentation standards.
  • Monitor patient care plans and outcomes, making recommendations for improvements as necessary.
  • Facilitate communication between primary care providers, hospitals, and other healthcare entities regarding patient status and needs.

Requirements

  • LVN or RN License
  • 1 year of experience as an MDS Coordinator
  • Strong understanding of medical management systems and processes
  • Knowledge of physiology, medical terminology, and patient care practices
  • Familiarity with medical records management and compliance regulations
  • Excellent communication skills
  • Strong organizational skills to manage multiple tasks effectively

Nice-to-haves

  • Experience in utilization review or similar roles within a healthcare setting
  • Previous experience in a Skilled Nursing Facility (SNF)
