Responsibilities
- Conduct comprehensive assessments of patients using the MDS framework to gather essential clinical data.
- Collaborate with nursing staff and other healthcare professionals to ensure accurate and timely completion of MDS documentation.
- Review and analyze medical records to ensure compliance with regulatory requirements and quality standards.
- Participate in utilization reviews to assess the appropriateness of care provided to patients.
- Maintain up-to-date knowledge of changes in regulations related to MDS and patient care practices.
- Provide training and support to staff on MDS processes, medical terminology, and documentation standards.
- Monitor patient care plans and outcomes, making recommendations for improvements as necessary.
- Facilitate communication between primary care providers, hospitals, and other healthcare entities regarding patient status and needs.
Requirements
- LVN or RN License
- 1 year of experience as an MDS Coordinator
- Strong understanding of medical management systems and processes
- Knowledge of physiology, medical terminology, and patient care practices
- Familiarity with medical records management and compliance regulations
- Excellent communication skills
- Strong organizational skills to manage multiple tasks effectively
Nice-to-haves
- Experience in utilization review or similar roles within a healthcare setting
- Previous experience in a Skilled Nursing Facility (SNF)
