About The Position

White Blossom Care Center is looking for a full time (RN) MDS coordinator. You may be our next hire to join our team! The MDS Coordinator assumes full time administrative authority, responsibility and accountability to coordinate the completion of the required minimum data set and with other members of the inter-disciplinary team, develops and implements a plan of care that meets the individual needs of each resident.

Requirements

• Current active license as a Registered Nurse (RN).
• At least 1 year experience in long term geriatric nursing care.
• Experience in completing the Minimum Data Set is preferred.
• Must be able to read, analyze, and interpret common scientific and technical information.
• Must be able to communicate effectively in the English language.
• Must have exceptional communication and customer service skills.
• Must be empathetic.
• Ability to effectively communicate with patients, families, responsible parties, staff and outside resources and agencies.

Responsibilities

• Coordinate the completion of the required minimum data set.
• Develop and implement a plan of care that meets the individual needs of each resident.
• Work with other members of the inter-disciplinary team.
