About The Position

The MDS Coordinator plays a critical role in ensuring the accuracy and compliance of resident assessments within the facility. This position is responsible for coordinating and completing the Minimum Data Set (MDS) assessments, which are essential for care planning, reimbursement, and regulatory compliance. The coordinator collaborates closely with interdisciplinary teams to gather comprehensive clinical data and ensure that resident care plans reflect current needs and conditions. By maintaining meticulous documentation and adhering to federal and state guidelines, the MDS Coordinator supports quality care delivery and optimizes facility operations. Ultimately, this role contributes to enhancing resident outcomes and sustaining the facility's reputation for excellence in healthcare services.

Requirements

  • Current and valid Registered Nurse (RN) license in the state of Indiana.
  • Experience with MDS assessment processes and knowledge of CMS regulations.
  • Strong clinical background in long-term care or skilled nursing facility settings.
  • Proficiency in electronic health records and MDS software systems.
  • Excellent organizational and communication skills.

Nice To Haves

  • Certification as an MDS Coordinator or completion of specialized MDS training programs.
  • Familiarity with quality improvement initiatives and regulatory survey processes.
  • Advanced knowledge of Medicare and Medicaid reimbursement methodologies.
  • Demonstrated ability to lead interdisciplinary teams and manage multiple priorities.

Responsibilities

  • Conduct and coordinate comprehensive MDS assessments for residents in accordance with CMS guidelines and facility policies.
  • Collaborate with nursing staff, therapists, and other healthcare professionals to collect accurate clinical information.
  • Review and analyze resident data to ensure completeness and accuracy of documentation.
  • Prepare and submit MDS data within required timeframes to support billing and regulatory compliance.
  • Provide education and training to staff regarding MDS processes and documentation standards.
  • Monitor changes in resident conditions and update assessments as necessary to reflect current status.
  • Participate in quality assurance activities and audits related to MDS documentation and compliance.
