COMMUNITY NAME Bedford Court Job ID 2025-236196 JOB OVERVIEW The MDS Coordinator is responsible for the overall accurate completion of the Resident Assessment Instrument (RAI) process for all guests/residents in the Skilled Nursing Centers. The Resident Assessment Coordinator (RAC) is responsible to be knowledgeable in the Sunrise skilled nursing philosophy of the Resident Centered Care model and serving our guests/residents in accordance with federal, state, and local laws and regulations to deliver and promote the highest degree of quality care and services to our guests/residents.

Requirements

  • Graduate of approved college/school of nursing
  • Maintains a current state/province license as a Professional Registered Nurse (RN) or Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN)
  • One (1) year supervisory experience in long term care environment which may include recruiting staff, coaching, performance management, and responsibility of daily department operations
  • Minimum Data Set (MDS) and Care Plan Development experience
  • Ability to complete the AAPACN (American Association of Post Acute Nursing) Resident Assessment Coordinator (RAC) certification
  • Ability to handle multiple priorities
  • Ability to delegate assignments to the appropriate individuals based on their skills, roles, and interests
  • Possess written and verbal skills for effective communication and the ability to facilitate small group presentations
  • Competent in organizational and time management skills
  • Ability to accurately assess and document assessments
  • Demonstrates good judgment, problem solving, and decision-making skills
  • Demonstration of proficiency in computer skills, Microsoft Office and Sunrise applications with the ability to learn new applications
  • Ability to work weekends, evenings, and flexible hours, available for our customers at peak service delivery days and times
  • As applicable, all Sunrise team members who drive a Sunrise vehicle must sign the Driver job description and understand the key essential duties for safety and regulatory compliance
  • Employment with Sunrise is contingent upon completing and passing a drug test (which does not include marijuana) and Tuberculosis Test, and a physical evaluation and a background check where required.
  • Covid-19 and Influenza vaccination may be required if mandated by applicable federal, state, and local laws and authorities.

Responsibilities

  • Coordinate accurate completion of the Resident Assessment Instrument (RAI) using information in the health information record, Activities of Daily Living (ADL), 24 Hour Report, and others as appropriate.
  • Complete Minimum Data Set (MDS) in accordance with Federal, State/Provincial, and Local policies, regulations, and guidelines.
  • Maintain schedule of the MDS, Care Area Assessment (CAA), Care Plans, and assessments.
  • Provides training and education to the skilled nursing clinical team members regarding MDS / Care Plan process and required documentation.
  • Facilitate the Triple Check and Medicare meetings weekly.
  • Ensure billing information is available for the Business Office to submit to Sunrise CSO on a timely basis.
  • Review clinical documentation in the guest/resident health information record routinely for accuracy and completeness.
  • Ensure CAA process and Care Plan development process are completed timely and accurately.
  • Review guest/resident’s Care Plans routinely ensuring individualization, accuracy, and completeness.
  • Ensure Individualized Care Plans meet the guest/resident’s medical, nursing, and psychosocial needs as identified in the guest/resident assessments and address the guest/resident’s personal goals.
  • Ensure documentation of guest/resident progress to goals are addressed on the weekly / monthly summaries.
  • Ensure skilled services documentation supports skilled services provided on a daily basis.
  • Proficient in International Classification of Diseases (ICD) 10 coding.
  • Respond to guest/resident, family, and visitor’s request in a timely and professional warm manner.
  • Maintain confidentiality of all guest/resident information.
  • Coordinate and utilize available health care resources.
  • Invite and encourage families, friends, and/or responsible parties to participate in the Care Plan process.
  • Communicate with families regarding medical care of the guest/resident through the Care Plan process.
  • Maintain knowledge of the Federal and Sunrise Quality Clinical Indicators.
  • Use CASPER/ iQUES database to download and review reports with Interdisciplinary Team (IDT).
  • Access and review publicly reported nursing home data with IDT.
  • Maintain the guest/resident roster/matrix (CMS 802) for all current residents.
  • Attend and actively participates in IDT and Quality Assurance Performance Improvement (QAPI) meetings and provides analysis of Quality Measures.
  • Provide analysis of the Quality Indicators as requested.
  • Assist with development of plans for Quality Indicators above the Sunrise threshold.
  • Maintain knowledge with the items on the MDS that trigger the Quality Indicators.
  • Assist with the Implementation of the Plan of Correction for federal survey.
  • Demonstrate proficiency and regular usage of MDS software, to include SCC (Sunrise Care Connect) and Point Right analytic program.
  • Demonstrate and knowledgeable in the following key regulations: All Federal, State/Province and Local resident care nursing regulations Resident Rights Resident Assessment Instrument process Actively participates in Federal, State/Provincial, Local surveys
  • Demonstrate and is knowledgeable of the Six (6) Rights of Medication Pass (“Right” resident, medication, dosage, time, route, right to refuse).
  • Maintain knowledge in the appropriate medication for appropriate diagnosis for the senior population.
  • Provide leadership and follow Sunrise and nursing standards for taking orders from physicians and administering medications.
  • Maintain knowledge of Restorative Practices of the Resident Assessment Instrument (RAI) process.
  • Lead and follow the documentation guidelines of the Restorative process.
  • Train and monitor the designate nursing assistants in the Restorative program.
  • Monitor the documentation for the Restorative program.
  • Monitor for resident decline/improvement in condition.
  • Refer resident/guests to restorative nursing as indicated.
  • Assist in the presentation and value of Sunrise’s products and services for our residents, families, team members, and targeted referral sources.
  • Understand and manage the department budget to include labor/labour and other expenses and its impact on the community’s bottom line.
  • Coordinate with the community team to achieve maximum staff economies and cross training when applicable.
  • Understand the internal cost associated with all Sunrise resident care programs.
  • Partner with community team to ensure community is in compliance with national/provincial regulations pertaining to occupational health and safety requirements and promotion of Risk Management programs and policies; adherence to safety rules and regulations.
  • Practice safety procedures at all times including Personal Protective Equipment (PPE), fire extinguishers, Safety Data Sheets (SDS), and Lockout Tagout procedures.
  • Partner in the delivery and participation in Sunrise University Training and self-study programs during the required timeframe.
  • Develop a working knowledge of federal and state/provincial regulations and ensures compliance through supervising and coaching team members.
  • Achieve the Team Member Engagement goals and actively lead in the Impact Planning sessions.
  • Monitor Skilled Nursing clinical team members to ensure individualized Care Plans are followed and guest/residents receive care and services as designed; report any deficient practices to Director of Nursing Services (DNS), the Executive Director (ED), and the Skilled Nursing Administrator (SNA).
  • Hold team accountable and correct actions when necessary and document.
  • Attend regular meetings; IDT, Stand Up, Cross Over, Department Head Meetings, Town Hall, Quality Improvement, and others as directed by the SNA.
  • Keep abreast of professional developments in the field by reading, attending conferences, and training sessions.
  • Organize and partner in the delivery of the daily/weekly/monthly educational and training program for all nursing team members.
  • Maintain compliance in assigned required training and all training required by state/province or other regulating authorities as applicable to this role to ensure that Sunrise standards are always met.
