The MDS Coordinator - RN at Elaine Center at Hadley is a full-time position responsible for overseeing the MDS (Minimum Data Set) team to ensure the timely and accurate completion of the MDS treatment assessment tool. This role is crucial in reviewing care plans and their delivery, focusing on various factors specific to patients and residents, including physical, cognitive, and socialization aspects. The MDS Coordinator plays a vital role in maintaining compliance with high standards of care, which is essential for the overall quality of healthcare provided at the facility. In this position, the MDS Coordinator will manage the overall process and tracking of all Medicare and Medicaid case-mix documents. The MDS Coordinator will also integrate information from various departments, including nursing, dietary, social services, restorative, rehabilitation, and physician services, to ensure that reimbursement is appropriate and reflective of the services provided.
