About The Position
The MDS Coordinator | LPN at Carnegie Village Rehabilitation and Health Care Center is responsible for the timely and accurate completion of the RAI process and care management from admission to discharge. This role ensures compliance with company policies, federal and state guidelines, and oversees the Minimum Data Set (MDS) processes. The position requires collaboration with the Director of Nursing and the VP of Clinical Reimbursement to coordinate clinical information systems and education.
