About The Position

Andover Forest Post Acute Care Center is seeking an MDS Coordinator LPN Licensed Practical Nurse to fill a full-time , salaried position in our skilled nursing facility. The MDS Coordinator LPN Licensed Practical Nurse duties will be making sure that all paperwork is completed, and the facility is receiving the maximum reimbursements entitled. You will collect data and gather information from staff and resident to create a detailed, accurate and comprehensive clinical assessment . Requirements

An active state LPN license is required.
Understanding of the MDS process: coordination, scheduling and completion.
SNF/long-term care experience.
