Requirements

Active Licensed Practical Nurse (LPN) license in good standing.
6+ months of experience as an MDS Coordinator required.
A detail-oriented, compassionate professional with a commitment to excellence.

Nice To Haves

Experience in Skilled Nursing/ Rehabilitation preferred.
RAC-CT or RNAC certification preferred.

Responsibilities

Oversee the coordination and participate in the completion of the Resident Assessment Instrument (MDS, CAA's, and Care Plan) in compliance with Federal and State regulations.
Monitor and document the management of Medicare and Managed Care residents, working closely with the interdisciplinary team.
Ensure accuracy in resident assessments to support reimbursement and high-quality care.
Drive innovation by implementing new ideas and process improvements.
Stay up to date with regulations and best practices to maintain compliance.
