About The Position

Longhorn Village is a Texas born and bred not-for-profit retirement community that guarantees independent living for our residents, plus a full continuum of on-site care that includes assisted living, memory care, skilled nursing, and rehabilitation. We are seeking a candidate with a strong positive attitude toward customer service to act as a MDS Coordinator. This person will have a genuine interest in geriatric care and be able to motivate while maintaining relationships with staff and residents. The right candidate will always strive to do the best job possible, be creative, and work towards accomplishing facility goals.

Requirements

  • Minimum Licensed Vocational Nursing degree.
  • MDS experience required.
  • Two years of experience working as a licensed vocational nurse including some medical/surgical experience and awareness of the needs of geriatric residents.

Responsibilities

  • Coordinate the RAI process for all residents, performing/assisting as needed with assessments related to the process.
  • Plan, organize, develop and maintain a complete MDS program, ensuring that all federal, state and local standards are adhered to.
  • Assist in indicator development, standards development, data collection, display and analysis, problem solving and reporting.
  • Coordinate the IDT for Care Planning, assuring the highest quality of care for residents is developed at and above CMS guidelines.
  • Timely MDS completion per RAI manual, giving special attention to completion of forms.
  • Timely completion of RAI process and RAP summary, identification of problems, goals, and approaches.
  • Proper and accurate response to regulation changes and evaluation of the Care Plan system.
  • Assure timely signatures are obtained.
  • Effectively communicate with nurses and IDT on the current status of Care Plans.
  • Respond timely and in writing to deficiencies in MDS audits.
  • Assist with admission process.
  • Consistently demonstrate hospitality behavior and the core values of quality, respect, service and integrity.
