About The Position

The MDS Care Plan Coordinator at Plantation Nursing and Rehabilitation Center is responsible for managing the Minimum Data Set (MDS) process. This includes reviewing pre-admission screens, completing admission care plans, determining assessment reference dates, and scheduling and coordinating assessments for all Medicare residents to ensure compliance with PPS guidelines and required time frames. The role involves conducting full and comprehensive assessments, coordinating resident care plan meetings with the Interdisciplinary Team, and ensuring timely completion and transmission of all MDS forms. Additionally, the coordinator is responsible for completing and submitting Medicare Verification of Funding forms, coordinating RUGS III level changes in collaboration with Therapy and Nursing, and communicating these changes to the Business Office Manager. The position also requires identifying potential discrepancies between therapy provided and RUG III claimed for Medicare A residents, running draft MDS forms, and actively participating in utilization review and therapy team conference meetings. The coordinator may also be required to participate in the Manager on Duty Program, which includes facility rounds, checking departments, getting reports from nursing supervisors, conducting facility tours for prospective admissions, and assisting with resident concerns and overall facility operations.

Requirements

  • Education: RN, BSN
  • Licensure: Licensed in the State of Florida
  • Work Experience: Three years in an LTC facility and experienced in MDS
  • Computer Literacy: Must know basic data entry
  • Ability to ambulate throughout the facility as needed to complete job responsibilities
  • Ability to sit for time required to complete paperwork duties required

Responsibilities

  • Reviews the Pre-Admission screen prior to admission to determine admission acceptability.
  • Advises Administrator of any questionable admissions.
  • Completes an admission care plan for the resident.
  • Determines the Assessment Reference Date at time of admission.
  • Advises the unit of the “look back” period and provides the unit with the Certified Nursing Assistant worksheet.
  • Schedules assessments for all Medicare residents to meet PPS guidelines, notifies appropriate staff assessment dates and assures that required time frames are met without exception.
  • Conducts the full assessment on or before Day 5.
  • Coordinates the completion of the Comprehensive Assessment on or Before Day 14.
  • Notifies the Interdisciplinary Team of the Resident Care Plan meeting.
  • Coordinates the Care Plan meeting.
  • Completes and faxes to the corporate office the Medicare Verification of Funding form in a timely manner (By Day 5 upon admission and upon RUGS III level change during stay).
  • Coordinates all RUGS III level changes throughout resident’s stay, working closely with Therapy and Nursing.
  • Communicates all reimbursement level changes to Business Office Manager as soon as pending level change is identified.
  • Identifies potential discrepancies between therapy provides and RUG III claimed on Medicare A residents.
  • Runs draft of all MDS’s prior to locking the MDS on all Medicare A residents.
  • Assures all MDS’s are transmitted within the required time frames.
  • Attends and actively participates in all utilization review and therapy team conference meetings.
  • Performs facility rounds, checking each department and getting reports from the nursing supervisors as Manager on Duty.
  • Assists with resolution of resident concerns and overall facility operations as Manager on Duty.
  • Conducts facility tours for prospective admissions as Manager on Duty.
  • Others responsibilities assigned.
