MDS/Care Plan Coordinator
About The Position
COME ENJOY THE RIDGECREST EXPERIENCE WITH GREAT FOOD, GREAT CARE AND GREAT SERVICE!!! -Must take call and be willing to work shifts on the floor -Must be a licensed RN or LPN -Completing and updating MDS forms on all patients in the care facility -Performing clinical assessments to gather information on residents -Developing care plans for patients to coordinate available services -Submitting updates and paperwork to maintain compliance -Conducting and attending meetings with other care team members -Investigating complaints or issues from residents and their families -Educating patients and their families -Understanding of Medicare and Medicaid requirements -Familiarity with the MDS assessment tool, including MDS coding knowledge -An understanding of medical terminology, nursing practices and patient care standards -Attention to detail to ensure precision in completing assessments and accurate data entry -Assessment, data collection and documentation skills -Written and verbal communication skills -Organizational skills -Comfort with electronic records Ridgecrest offers excellent benefits!! Requirements
