About The Position

SUMMARY: The Care Navigator, under the general direction of the Nurse Care Manager or other licensed clinical team member, provides patient and practice support to enhance patient engagement in meeting care plan goals. Responsible for closing care gaps and monitoring general population health for the practice. The core Success Factors include: Instill Trust and Value Differences Patient and Community Focus and Collaborate RESPONSIBILITIES: Performs tasks related to collecting patient histories, medication reconciliation, or other medical information necessary for the nurse care manager or licensed clinician to effectively triage patient needs.Identify and assist patients who need to be referred to other team members including nurse care manager, social worker, pharmacist, or PCP.Conducts patient outreach and engagement via telephone, in person or via patient portal to assist nurse care manager or another clinical team member.Assists and schedules patients for physician appointments and coordinates transportation needs.Participates in chart audits to assist practice in meeting quality measures.Monitors and assists department with pre-visit planning activities including lab and referral tracking.Prospectively and retrospectively works to identify and close gaps in care.Performs outreach to patients to ensure gap closure and testing has been completed.Assists nurse care manager with transitions of care including monitoring utilization reports and conducting patient outreach when appropriate.Develops strong relationships with patient care team to work on quality improvement activities.Builds strategic relationships with outside organizations and providers to help support patients and families.Performs other duties as assigned.

Requirements

  • High school diploma and three to five years of clinical patient care experience.
  • Thorough knowledge for registration and ordering of lab and diagnostic tests.
  • Ability to represent the organization and serve consumers in a professional manner and promote a positive image of the organization and its services.
  • Interpersonal skills demonstrating an ability to communicate with multiple providers.

Nice To Haves

  • Nursing or LPN/Clinical experience preferred.
