POSITION SUMMARY: In this contractor role, the MD will be responsible for establishing the TapestryHealth patient relationship for our chronic care management services . We have openings in and around all major and rural cities.
Requirements
- Qualified MD/DO
- Active , unrestricted, Medical License(s) in California and multiple other states.
- Excellent clinical jud gment and decision-making skills.
- Must have reliable transportation to travel to assigned buildings.
- Commitment to providing patient centered care and improving health outcomes.
- Familiarity with electronic health record ( EHR ) systems and healthcare technology.
- Ability to travel within 1- 2 hours of assigned region
Responsibilities
- Responsible for establishing the TapestryHealth patient relationship for our chronic care management services .
- Availability 10+ weekdays per month to conduct approximately 3 75 clinical visits onsite and remotely.
- These are brief, educational consults simply advising patients of the services they are enrolled in.
- Tapestry Health will reimburse for mileage as well.
- Document patient encounters accurately and thoroughly within our platform at time of visit .
- Provide end of day report to program administrators to track progress for completion of visits.
