We are looking for a dedicated Maternal Child Health RN to join our team. This role involves providing compassionate, culturally competent care to diverse families and children while excelling in delivering high-quality perinatal and pediatric nursing services. The ideal candidate will be committed to upholding the highest standards in family and child health clinical practice and providing family-centered, safe, and effective care.

Requirements

  • Associate degree in Nursing; BSN program preferred.
  • Preferred one year experience as an RN in Maternal Child Health. New graduates or those without Maternal Child Health experience will be considered.
  • Valid VT Driver’s License and reliable transportation.

Responsibilities

  • Develop and share a personalized care plan for each client, incorporating input from the client, their family, and their doctor.
  • Collaborate with a multidisciplinary team and with community members to ensure comprehensive care and it complies with all regulations and agency guidelines.
  • Coordinate and deliver pediatric palliative care services in home settings, including psychosocial support and care planning.
  • Facilitate pediatric palliative interdisciplinary team meetings and care conferences to align goals of care.
  • Support our work with the Strong Families of Vermont Home Visiting programs.
  • Execute care plans, implement nursing interventions, and provide client and family education.
  • Communicate clearly and promptly with the healthcare team and keep accurate electronic records to support coordinated care.
  • Participate in on-call duties, including weekends and holidays, as needed by the agency.
