The Administrative Supervisor will serve as the central point of contact with the County and will supervise the daily operations of Medical Care Coordination and Patient Support Services Programs. The supervisor must function separately from the Patient Support Services (PSS) and Medical Care Coordination (MCC) teams. This role provides administrative oversight of the program, including but not limited to progress reports, program evaluation, staff training, and supervision. Medical Care Coordination Services utilize a multi-disciplinary team approach to facilitate behavioral interventions, conduct outreach, and coordinate support services to improve health outcomes for people living with HIV (PLWH). MCC team members deliver trauma-informed, client-centered services that address social determinants of health (SDH), enhance engagement in care, and coordinate health and support services for Ryan White Program (RWP). Patient Support Services provide interventions aimed at addressing behavioral, emotional, social, and environmental factors that negatively impact health outcomes for Ryan White Program (RWP) eligible clients. The goal is to enhance an individual's health, functioning, and overall well-being. PSS services are delivered by a multi-disciplinary team of specialists who conduct client-centered interventions focused on psychological and social determinants of health (SDH) to help clients remain engaged in HIV care, improve their health outcomes, and achieve or maintain viral suppression.
