About The Position

The Maternity Care Coordinator at VIVA HEALTH is responsible for providing non-clinical services to Medicaid recipients, focusing on improving medical compliance and health outcomes. This role involves identifying barriers to care, analyzing community environments, and developing appropriate care plans. The position requires a combination of psychosocial and clinical knowledge to support individuals in their healthcare journeys, primarily through community, clinical, and home settings.

Requirements

  • Licensed BSN or ADN
  • Licensed Social Worker
  • Experience in case management, human services, public health, or with the underinsured population
  • Maternity care coordination experience
  • Valid driver's license in good standing
  • Willingness to submit to vaccine testing and screening
  • Excellent interview and telephone skills
  • Ability to deal with eligible recipients in a caring and helpful manner
  • Working knowledge of health-related service delivery systems
  • Excellent communication and relationship skills
  • Ability to analyze varied environmental factors concerning the recipient's well-being
  • Ability to work independently in an autonomous setting.

Nice To Haves

  • Licensed Master of Social Work (LMSW) designation
  • Certified Case Manager (CCM) designation

Responsibilities

  • Provide non-clinical services for Medicaid recipients to improve medical compliance and health outcomes.
  • Identify barriers to medical compliance such as lack of transportation and illiteracy.
  • Ensure services are delivered and continuity of care is maintained.
  • Analyze home and community environments to make decisions regarding care plans and goals.
  • Utilize available community resources to support recipients' needs.
  • Conduct services primarily in community, clinical, and home settings via phone and in person.
  • Travel to various locations within the service area as needed.
