Performs MIHP and/or Behavioral Health office/home/telehealth encounters based on scope of practice limitations and program implementation needs. Completes and submits required documentation for patient record in compliance with MIHP/MDHHS and GLBHC (Great Lakes Bay Health Centers) standards and protocols. Complies with billing record keeping requirements of the program. Follows goals set by administration for productivity and financial sustainability for both self and staff. (10%) Provides direct supervision of MIHP staff and successful program implementations. Evaluates performance of staff to ensure following the organization guidelines and expectations. Facilitates MIHP team care plan development and monitoring through case conference with all disciplines working with client to include other agencies involved with the client. Arranges follow up with other team members Conducts employee evaluations promptly, monitors performance, trains, counsels, and implements disciplinary measures when needed. Continually assesses staff competency and coaches’ staff to maximum performance. Arranges and monitors orientation and training for new hire staff and providers. Responsible for maintaining all staff required documentation in staff files and completed training documentation. (25%) Responsible for ensuring program required guidelines are met. Assist with the development of MIHP required protocols based on cycle requirements, billing audits, and chart audits. In coordination with the Director of Women’s Health, organize and assist with the completion of audits through MDHHS. Responsible for ensuring that records are retained for at least 7 years with all remaining paper charts. This individual is also responsible for the successful understanding and use of the MIHP electronic medical record by all MIHP staff. Responsible for disseminating information to designated program staff. Responsible for disseminating all MIHP State Level communications and communications from MIHP consultants to MIHP staff. Shares all communications from other community programs that collaborate to serve MIHP (25%) Program participants. Attends all MDHHS MIHP Coordinator meetings and Centering Program Implementation committee meetings. Serves as liaison with CHI Centering Advisor and MDHHS MIHP Consultant for startup, ongoing maintenance and problem solving, and accreditation. (5%) Key member of Centering program implementation committee; assists with developing agendas and setting schedules for centering committee meetings; communicates with staff on the centering committee work. Collaborates with facilitators and Old Town Women’s Care OB/GYN Nurse Manager to develop group session schedules. Manages logistical issues for Centering groups including the ordering of supplies and materials, snacks, and assessment equipment/supplies. Monitors the Centering enrollment process to ensure optimal group size. (10%) Serves as a champion of the Women’s Care vision and liaison with GLBHC staff and other organizations. Collaborates and provides outreach to develop positive relationships with community organizations to adequately address community needs. Works effectively with other Women’s Care leaders and the department to carry forward initiatives and quality protocols. Connects with the public and other community organizations through representation at outreach events. Actively looks for opportunities to address patient and community needs around assigned programs. Provides outreach to other departments and community stakeholders and agencies to educate others about MIHP and Centering Pregnancy. Oversees and/or conducts the ongoing data collection and reporting for Centering Counts evaluation of the clinical setting performance measures and patient health outcomes. (20%) Ensures delivery of excellent customer service and addresses patient complaints/grievances professionally. (5%) MARGINAL JOB DUTIES Participates in special community activities throughout the service area to improve birth outcomes, decrease infant mortality, expand outreach, and increase community involvement. Directly assists the Director of Women’s Health with other projects and duties. Other job duties as assigned.
