About The Position

Jericho Road Community Health Center is actively seeking a Maternal Health Ambassador. This position is full-time, within the Community Health Department, at our West Ferry location. Requirements

  • Lived experience in and strong ties to Buffalo’s East Side community
  • Deep familiarity with local community resources, organizations, and leaders
  • Demonstrated interest in maternal health, community health, and/or primary care
  • Strong interpersonal, communication, and advocacy skills
  • Ability to work independently and collaboratively as part of an interdisciplinary care team
  • Basic computer skills, including use of office equipment, word processing, spreadsheets, EHR systems, and other software applications
  • Willingness and ability to complete Community Health Worker, doula, and Certified Child Passenger Safety Technician training programs upon hire

  • Prior experience working in maternal health, community health, primary care, or related community-based settings preferred

Responsibilities

  • Serve Black women living on Buffalo’s East Side, prioritizing outreach and engagement in zip codes 14206, 14210, 14211, 14212, and 14215, in partnership with the Buffalo Center for Health Equity
  • Represent JRCHC at community events, health fairs, schools, places of worship, and local businesses
  • Assist staff with program evaluation through surveys, feedback collection, and quality improvement cycles
  • Provide culturally appropriate health education on preconception health, prenatal care, postpartum care, breastfeeding, chronic disease management, behavioral health, and preventive health to target population
  • Develop personalized care plans that include collaborative goalsetting and shared decision making
  • Promote self-sufficiency in meeting short- and long-term needs and goals with patients
  • Complete social determinants of health screenings and complete resource navigation
  • Assist patients with care coordination, appointment scheduling, referrals, transportation navigation, and follow-up
  • Facilitate connections to culturally representative doulas and community-based supports
  • Support the economic and educational advancement of Black women by connecting them with resources for GED/training/educational programs, linking them to financial literacy programs, and completing referrals to home buying programs
  • Work closely with JRCHC clinicians, behavioral health providers, doulas, and CHWs
  • Serve as a trusted representative for participants, ensuring their voices, concerns, and preferences are heard and respected within healthcare and social service systems
  • Assist with facilitation of quarterly Community Advisory Board meetings and weekly peer support groups
  • Support broader maternal health equity efforts through community education, storytelling, and engagement in advocacy initiatives as appropriate
