About The Position
Virginia Mason Franciscan Health is seeking a dedicated Masters Social Worker to join our team in Silverdale, WA. This role is integral to our mission of providing high-quality, compassionate care to our patients. The Masters Social Worker will be responsible for conducting social work assessments and interventions for patients in the hospital and emergency department settings. This includes crisis intervention, high-risk screening, brief counseling, and facilitating family meetings with the healthcare team. The social worker will also arrange post-acute placements for complex discharges and engage with community resources for high-risk patients. In this position, the Masters Social Worker will provide developmentally appropriate care for all populations, ensuring safe discharge and continuity of care. The role requires advocacy and education for patients and families, focusing on self-care management and health management education. The social worker will also be involved in psychosocial management, addressing issues such as abuse, neglect, and emotional adjustment, while providing grief and bereavement support. Additionally, the social worker will coordinate care conferences and manage transitions of care, ensuring that patients receive the necessary support and resources. The Masters Social Worker will also play a crucial role in community resource coordination, helping patients navigate healthcare resources and support programs. This includes building relationships with community organizations to address social determinants of health, such as housing and food insecurity. The position requires a thorough understanding of regulatory guidelines and the implementation of evidence-based practices to support organizational goals and improve patient outcomes. The social worker will also participate in performance improvement initiatives and adhere to all hospital policies and regulations.
