About The Position

Center for Human Development, (CHD), is currently looking for a Master's level, licensed preferred Clinician to join the team at Heron House, an Enhanced Medical Group Living Environment (EMGLE) program. This brand new 18-bed facility located in East Longmeadow, MA allows you the ability to provide quality care and support to those we serve. The Enhanced Medical GLE (EMGLE) provides a variety of skilled healthcare and supportive services including around the clock nursing and personal care assistance to Persons with mental illness in addition to compounding medical needs. These services are designed to meet and support the daily needs of Persons with chronic medical conditions, terminal illnesses and/or disabilities, which are impacted by their significant mental illness. These Persons require licensed and unlicensed professional services to meet their chronic complex medical and behavioral needs to ensure improvement or stabilization of their medical and behavioral condition. This model will also serve to maintain, prevent, or slow worsening of condition as well as assisting transition to a lower level of care setting or prevent hospitalization/nursing facility admissions. A competitive hourly wage of $31.00 and an attractive benefit package are available with this Master's level, licensed preferred Clinician opportunity.

Requirements

  • Clinician holds a Master's in psychology, social work or related field from an accredited institution.
  • Eligible for credentialing by Mass Health.
  • Clinicians eligible for licensure will provide a copy of their professional license and must renew/maintain their license in good standing.
  • Clinicians will complete and update any documentation required for the credentialing process.
  • Proficient in verbal and written communication.
  • Able to utilize a computer and become competent in the use of an electronic health record.

Nice To Haves

  • Strong written skills and experience with Electronic Health Record Management.

Responsibilities

  • Attend participants care plan meetings/resource meetings.
  • Provide diagnostic evaluations, treatment planning and outpatient psychotherapy.
  • Develop a list of needs based on medical necessity, along with objectives and interventions to improve level of functioning and quality of life.
  • Learn and practice skills in evidence-based modalities.
  • Participate and successfully complete training requirements in compliance with CHD and the Massachusetts Department of Mental Health.
