About The Position

The position provides psychosocial services to patients treated by the facility, including in-center and home dialysis patients, utilizing Social Work Theory of Human Behavior and accepted methods of social work practice. The role involves working with the healthcare team to promote positive adjustment, rehabilitation, and improved quality of life for patients. The social worker collaborates with the interdisciplinary team to inform, educate, and support staff in understanding the emotional, psychological, and behavioral impact of Chronic Kidney Disease on patients and their families. This ensures comprehensive quality care and supports the FMCNA commitment to Quality Indicators and Outcomes and Quality Assessment and Improvement (QAI) Activities, including those related to patient satisfaction and quality of life.

Requirements

  • Masters in Social Work.
  • State required license.
  • 2 – 5 years of related experience.

Responsibilities

  • Assess patients' psychosocial status, strengths, and areas of need that may affect rehabilitation and treatment outcomes.
  • Participate in care planning in collaboration with the patient and healthcare team.
  • Utilize patient education programs and social work interventions to address barriers and meet treatment goals.
  • Provide monitoring and interventions for patients to adjust to dialysis and achieve optimal psychosocial status.
  • Offer supportive counseling services to patients within the scope of clinical training and state license.
  • Assist patients with referrals to community resources to facilitate optimal treatment outcomes.
  • Assess patient awareness of advance directives and facilitate discussions regarding DNR status.
  • Document interactions and interventions provided to patients and families.
  • Participate in monthly Quality review meetings and report on quality indicators related to adherence.
  • Provide education tailored to patients' learning styles and needs regarding kidney disease and treatment modalities.
  • Facilitate the transplant referral process and manage transplant waitlist in collaboration with the team.
  • Review patient rights and responsibilities with patients and their representatives.
  • Support the clinic in scheduling to maximize adherence and adjustment for patient admissions.
  • Collaborate with Insurance Coordinators to address insurance-related issues and educate patients about payment options.
  • Assist with the interview process for new personnel if requested.
  • Provide training to staff on psychosocial topics as needed.
