• Familiarity with aging issues counseling and geriatrics is desirable.
• Strong understanding of HIPAA regulations and ethical standards in social work practice.
• Excellent communication, leadership, and interpersonal skills.
• Ability to work collaboratively within a multidisciplinary team environment.

Responsibilities

• Ensure compliance with HIPAA regulations and organizational policies for all services provided.
• Assess and evaluate patients' needs for community resources and in-home support services.
• Assist in implementing the plan of care and support patients with advance directives.
• Maintain accurate clinical notes and documentation of client interactions and treatment plans.
• Oversee the development and implementation of treatment plans for home health patients, focusing on aging issues counseling and geriatrics.
• Collaborate with other healthcare professionals to ensure holistic patient care.
• Address any issues or concerns related to client care promptly and effectively.
